The Mastery in Therapeutic Endoscopy & Colonoscopy program is a specialized hands-on fellowship designed for General Surgeons and Gastroenterologists. This course bridges the gap between basic diagnostics and advanced therapeutic procedures. Trainees get direct exposure to live clinical cases, high-definition simulation, and proprietary STAR protocols for early detection and mucosal resection.

Course Features & Methodology Day to Day Schedule

Day 1: Physics, Port Placement & Ergonometrics

Morning Session – Induction & Technical Foundation

  • The Laparoscopic Tower: Understanding the light source, insufflator, camera processor, and CO2 management.

  • Pneumoperitoneum: Mastering safe entry techniques (Veress Needle vs. Hasson’s Open Method).

  • The Physics of Fulcrum: Understanding how the abdominal wall acts as a pivot and its impact on instrument movement.

  • Ergonomics: Proper table height and monitor positioning to prevent “Surgeon’s Neck” and fatigue.

Afternoon Session – The Precision Endotrainer Lab

  • 2D to 3D Transition: Exercises to overcome the loss of depth perception.

  • Hand-Eye Coordination Drills: “Peg Transfer” and “Pattern Cutting” to build muscle memory.

  • Ambidexterity Training: Developing the non-dominant hand for retraction and assistance.

Theory Session – The Core Pillars

  • Energy Sources in Laparoscopy: Monopolar, Bipolar, and Ultrasonic shears (Harmonic) safety.

  • Complication Management: Immediate recognition of vascular or visceral injuries during entry.

  • Patient Selection: Contraindications for laparoscopy and when to “convert to open” surgery.

Day 2: Triangulation & Essential Procedures

Morning Session – Live OT Observation

  • Diagnostic Laparoscopy: Systematic inspection of the peritoneal cavity.

  • Triangulation Principles: How to place ports to avoid “sword fighting” with instruments.

  • Live Case Flow: Observation of Laparoscopic Appendectomy and Cholecystectomy (Gallbladder removal).

Afternoon Session – Applied Mastery Workshop

  • Clip Application: Mastering the “C” and “U” maneuvers for cystic duct and artery ligation.

  • Dissection Techniques: “Cold” vs. “Hot” dissection and identifying the “Critical View of Safety.”

  • Specimen Retrieval: Using endobags and port-site management.

Day 3: Suturing Mastery & Certification

Morning Session – Translational Bio-Skills Lab

  • The “Gladiator” Technique: Mastering needle loading and orientation in the needle holder.

  • Intracorporeal Knot-Tying: Step-by-step drills for the “C-Loop” and “Reverse-C” square knots.

  • Extracorporeal Knotting: Using a Roeder’s knot and pushers for high-tension closures.

  • Tissue Handling: Practicing on biological membranes to learn the “feel” of tension and tear-resistance.

Afternoon Session – Assessment & Convocation

  • The FLS Competency Test: Timed performance of peg transfer, circle cutting, and one intracorporeal knot.

  • Individual Performance Review: Direct feedback from faculty on your “economy of motion.”

  • Convocation Ceremony: Awarding of the Certificate in Fundamentals of Laparoscopic Surgery.

  • Closing Remarks: Your roadmap to Advanced Laparoscopy (Hernia, Bariatric, and Colorectal).
