The Mastery in Therapeutic Endoscopy & Colonoscopy program is a specialized hands-on fellowship designed for General Surgeons and Gastroenterologists. Day 1: Advanced Diagnostics & The Science of Proctology

Morning Session – Induction & Pathophysiology

  • Clinical Orientation: Understanding the global burden of Colorectal diseases.

  • The “Complex” Definition: Criteria for identifying complex vs. simple Fistulas and recurrent Hemorrhoids.

  • Advanced Imaging: Interpreting MRI Defecography and Endo-Anal Ultrasound (EAUS).

  • Instrumentation: Setup of Video-Proctoscopes and High-Frequency Radiofrequency (RF) units.

Afternoon Session – The Precision Bio-Skills Lab

  • Mapping the Tract: Hands-on simulation of fistula tract tracing on advanced models.

  • Energy Sources: Understanding the physics of Laser (Diode), RF, and Harmonic Scalpels.

  • The “Bloodless Field” Technique: Practicing ligation and cautery in confined spaces

Theory Session – Mastering the "Gold Standards"

  • Hemorrhoidal Excellence: Comparative study of MIPH (Stapled), HAL-RAR, and Laser Hemorrhoidoplasty (LHP).
  • Fistula Strategies: Deep dive into LIFT, VAAFT, and Sphincter-saving procedures.
  • Emergency Proctology: Managing Perianal Abscesses and Fournier’s Gangrene.

Day 2: Live Surgical Exposure & Laser Integration

Morning Session – Live OT Exposure (Complex Cases)

  • Theatre Protocol: Scrubbing for advanced proctology and patient positioning (Lithotomy vs. Prone Jackknife).

  • Real-Time Intervention: Observing Laser Sphincterotomy and Complex Fistulotomy with Seton placement.

  • The “Intra-Operative Pivot”: Learning when to change the surgical plan based on unexpected findings.

Afternoon Session – Applied Mastery Workshop

  • Laser Fiber Handling: Calibration and safe usage of 1470nm Diode Lasers.

  • Stapler Application: Hands-on practice with PPH (Procedure for Prolapse and Hemorrhoids) staplers.

  • Advanced Wound Care: Techniques for vacuum-assisted closure and biological glues in Proctology.

Day 3: Translational Skills & Clinical Convocation

Morning Session – Ex-Vivo Translational Lab

  • Tissue Interaction: Performing LHP (Laser Hemorrhoidoplasty) and FiLaC (Fistula-tract Laser Closure) on animal tissue models.

  • Sphincter Repair: Simulation of Anal Sphincteroplasty and management of fecal incontinence.

  • Complex Flap Reconstruction: Practicing Advancement Flaps (Endorectal/Anocutaneous) for high fistulas.

Afternoon Session – Advanced OT & Professional Certification

  • Assisted Scrub-In: Participation in complex re-do surgeries or multi-tract fistula cases.

  • The “Mastery Assessment”: Final evaluation of surgical logic, instrument choice, and safety protocols.

  • Convocation Ceremony: Distribution of the Fellowship in Advanced Coloproctology Certificate.

  • Practice Growth: Guidance on setting up a dedicated “Proctology Excellence Center.”
