Foundations of Coloproctology

The Mastery in Therapeutic Endoscopy & Colonoscopy program is a specialized hands-on fellowship designed for General Surgeons and Gastroenterologists. This course bridges the gap between basic diagnostics and advanced therapeutic procedures. Trainees get direct exposure to live clinical cases, high-definition simulation, and proprietary STAR protocols for early detection and mucosal resection.

Course Features & Methodology Day to Day Schedule

Day 1: Anatomy, Examination & Basic Pathologies

Morning Session – Induction & Clinical Assessment

  • Functional Anatomy: Deep dive into the anal canal, sphincters, and the dentate line.

  • The Art of Examination: Proper technique for Digital Rectal Examination (DRE) and Proctoscopy.

  • Patient Communication: How to conduct sensitive consultations for anorectal disorders.

  • Documentation: Clinical grading of Hemorrhoids and classification of Fistula-in-ano (Park’s Classification).

Afternoon Session – Diagnostic Skills Lab

  • Proctoscope Handling: Mastering the insertion and visualization of the anal cushions.

  • Basic Instrumentation: Introduction to the “Proctology Tray” (Speculums, Probes, and Forceps).

  • Biopsy Techniques: Safe practices for rectal and anal mucosal sampling.

Theory Session – The Core Pillars

  • Hemorrhoids 101: Conservative management vs. Office-based procedures (Sclerotherapy/Banding).

  • Anal Fissures: Pathophysiology and the shift from medical management to Lateral Internal Sphincterotomy (LIS).

  • Abscess Management: Early diagnosis and the “Incise & Drain” gold standard.

Day 2: Essential Procedural Training

Morning Session – Live OT Observation

  • OT Positioning: Setting up the Lithotomy and Sims’ positions.

  • Anesthesia Selection: Understanding Local vs. Saddle blocks for minor proctology.

  • Live Case Flow: Observation of simple Hemorrhoidectomy, Fissurectomy, and simple Fistulotomy.

Afternoon Session – Simulation & Skill Drills

  • Rubber Band Ligation (RBL): Practicing band placement on simulated internal hemorrhoids.

  • Suturing in the Anal Canal: Exercises on knot-tying in confined, high-tension spaces.

  • Probe Navigation: Safe passage of a fistula probe without creating “false passages.”

Day 3: Applied Skills & Clinical Integration

Morning Session – Bio-Skills Lab (Basic Interventions)

  • Tissue Handling: Practicing basic excision and cautery techniques on tissue models.

  • Absure & Hemostasis: Managing minor post-operative bleeding in a simulated environment.

  • Wound Dressing: Modern protocols for open proctology wounds and sitz bath counseling.

Afternoon Session – Assessment & Certification

  • Clinical Competency Test: A “mock clinic” where trainees diagnose cases and suggest treatment plans.
  • Final Feedback: One-on-one session with faculty to identify areas for improvement.
  • Convocation: Awarding of the Foundations of Coloproctology Certificate.
  • The Roadmap: Introduction to the Advanced Coloproctology and Laser modules.
